**Before registering, please take a few moments
and read the following instructions carefully:
Instructions:
- Make sure to download the necessary forms to your computer. Simply "right click" on each form and then click "save as" and you will be given the option of where you want to save each form.
- Fill in the required information below
- Once you have completed the form, click "complete registration" button
- Once you click the submit button, you will be taken to the FBC Store page where you will be able to pay for camp. You can pay with debit, credit, or paypal account.
- Get both of the medical waivers notarized (you can do that at the FBC church office) and turn those in to Shawna Ashburn (Junior High) or Mary Koym (High School)
**please note: your child will not be allowed to travel with us to camp unless
both of the medical release forms are filled out, signed, and notarized.
